Symptoms may include fatigue, breathlessness, muscle or joint pain, dizziness, sleep problems and difficulty concentrating. They can improve, return or change over time.

Rehabilitation should be personal. A standard fitness programme or advice to push through symptoms may not be suitable, particularly when activity causes a delayed symptom flare.

Speak to your GP first

The NHS Long COVID guidance advises speaking to a GP if you think you may have Long COVID. Assessment may be needed to check your symptoms and rule out other causes.

Seek urgent medical help for severe breathing difficulty, chest pain, fainting, new confusion, signs of stroke or another sudden deterioration.

What is pacing?

Pacing means balancing activity and rest so that daily demands stay within what your body can currently manage. Activity includes physical tasks, concentration and emotional demands.

A common pattern is doing a great deal on a better day, followed by a marked increase in symptoms and a need for prolonged rest. Pacing aims to reduce that cycle.

Practical steps can include:

keeping a simple activity and symptom diary

breaking larger jobs into smaller parts

planning rest before becoming exhausted

spreading demanding tasks through the day or week

prioritising essential activities

changing how a task is done to use less energy

Leeds Teaching Hospitals describes the three Ps of pacing as pace, prioritise and plan. Its guidance warns that increasing activity in a way that worsens symptoms needs to be avoided.

Post-exertional symptom exacerbation

Some people experience post-exertional symptom exacerbation, sometimes called PESE or PEM. Physical or mental activity may cause symptoms to become worse hours or days later. The response can last for a day or longer.

If this happens, exercise progression must be cautious and guided by symptoms. Repeatedly increasing activity despite a delayed flare is not a suitable rehabilitation strategy.

How physiotherapy may support recovery

Depending on your assessment, physiotherapy may help with:

breathing control and positions for breathlessness

safe movement after a period of reduced activity

gentle strength and mobility work

activity planning and pacing

monitoring the response to activity

balance or dizziness-related rehabilitation where appropriate

working towards personal goals without fixed recovery promises

For some people, the first goal may be managing basic daily tasks more consistently. Exercise should only be introduced or progressed when it is appropriate for the symptoms and medical advice.

Recovery goals can be flexible

Goals may involve washing and dressing with less fatigue, preparing a meal, taking a short walk, returning to caring responsibilities or planning a phased return to work. Progress may be uneven, so the plan should be reviewed rather than forced.

Ask about Long COVID rehabilitation

Crestwell Medical provides Long COVID and respiratory rehabilitation, including tele-rehabilitation where suitable. Book an appointment to discuss your symptoms and goals.

This article is general information and does not replace assessment by a GP or specialist Long COVID service.

Understand how pacing, symptom tracking and personalised physiotherapy may support people living with fatigue, breathlessness and reduced strength after COVID-19.

[17

post-3627","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-updates
["GET
